Request by Caltrans to amend permit granted for replacement of Hwy 101 Mad River Bridge for: (1) approval of comprehensive long-term compensatory fisheries mitigation plan, in satisfaction of Special Condition No. 5D; and (2) modifications to requirements of Special Condition No. 15C to reduce required 4:1 mitigation ratio to 3.4:1 and change location of offsite mitigation, at Highway 101 crossing Mad River, between Arcata and McKinleyville, Humboldt County. b.  Permit No. 1-09-020-A2 (City of Arcata Habitat Restoration Project). Request by City of Arcata to amend permit granted for habitat restoration to authorize installation of 3 small cattle crossing bridges to replace former culverts on Fickle Hill Creek, located between Union St., Old Arcata Rd. and Hwy 101, Arcata, Humboldt County.
